Change details of a food business
You need to tell the Health Protection Service within 7 days of any changes to your food business.
Changes include:
- phone numbers or contact details
- business address
- business name
- risk classification
- ownership
- renovations and fit-outs.
How to apply
There are two ways to apply:
- complete the online food business variation or transfer form
- or ask to be emailed a form to print, fill out and return. Contact hps@act.gov.au or call 02 5124 9700.
