Better care in the home for Older Canberrans

A trip to the hospital can be difficult for some older people living in aged care but the Geriatric Rapid Acute Care Evaluation (GRACE) will soon change that.

GRACE makes it easier and more comfortable for seniors to be treated for free, in a place where they’re familiar—without taking a trip to hospital and reducing demand on the hospital emergency department.

Following a successful trial in 2016 on the northside of Canberra, over the next 3 years, all residential aged care facilities in Canberra will have the opportunity to participate in the service.
