If your wellbeing is at very high risk that is unusual for you  we may be able to help.  You may be eligible to get help for a short time from our program. Our program is called the Integrated Service Response Program (ISRP). We may be able to help you by:

  • coordinating with support services and the National Disability Insurance Agency (NDIA)
  • providing some funding for your disability services for up to 12 weeks.

Check if you can get help

You must:

  • be an ACT Resident
  • be an active participant in the National Disability Insurance Scheme (NDIS)
  • have no other way to resolve your support needs.

You might qualify for our help if you have a disability that fits NDIS rules. But, you can't get it because of your visa or your citizenship.

What supports you might get

If we fund supports for you, the supports must be delivered through a registered NDIS provider.

Depending on your circumstances, we may fund disability related supports for a short time to respond to your needs.

What you cannot get

You cannot get help to pay for:

  • rent and other ongoing housing costs
  • medical costs and diagnostic assessments
  • ongoing case coordination
  • ongoing disability supports that the NDIS must provide
  • disability supports that the health or education systems must provide.

How to get help

You should contact someone who can assess your need for urgent help and make a referral to us. We call this person your referrer. Your referrer can be:

  • an NDIS support coordinator
  • a health professional
  • a disability services provider
  • a housing worker
  • a  Children, Youth and Families worker
  • a person from the Office of the Public Advocate .
  • a person from education.

The referrer cannot be your parent or guardian.

The referrer who is helping you must:

  1. Download and fill in the Integrated Service Response referral form [PDF 585kB].
  2. Get you, your parent or legal guardian to sign the consent section.
  3. Email the form to the Integrated Service Response Program at isrp@act.gov.au

We will need to work closely with your referrer.

Giving your consent

Your referrer will ask you, or your parent or legal guardian, to sign the referral form. This proves you give your consent.

This means you agree for us to share information about you to get you the support you need.

We may share your details with other services such as:

  • Housing ACT
  • the National Disability Insurance Agency
  • government services like housing, health, mental health, child protection and education.

What happens next?

It might take up to two weeks for us to assess the referral. During this time we will organise a meeting with your referrer and other support services. We will work together to make a plan on how we may help you.

After the meeting your referrer will contact you to let you know what the plan is. We will make sure everyone works together to support your needs.
