Healthcare facility proprietors must produce an annual report for each of the premises they operate.

What you need to report

An annual report must contain the following:

  • name and location of the facility
  • number of professional staff
  • number of non-professional staff
  • number of patients admitted
  • number of notifiable incidents
  • number of births and deaths at the facility
  • details of the facility’s accreditation to the NSQHS Standards
  • the number and nature of complaints received.

The reporting period is the previous financial year.

Licensed healthcare facilities must use the Healthcare facility annual report template [DOC 127 KB].

When and how to submit

You must submit your annual report for the previous financial year by 30 September.

If the facility opened less than 12 months before 30 June you don’t need to report for that year.

There are 2 ways to submit your annual report:

  • email hps@act.gov.au
  • mail to: Health Protection Service, Locked Bag 5005, Weston Creek ACT 2611.
