Funding to project manage the delivery of an after-hours palliative care medicines service

Palliative Medicines in Community Pharmacy (PMCP) Project Grant
This grant supports the establishment of an after-hours palliative care medicines service in the ACT. The service will enable greater access to palliative medicines and pharmacy support for Canberrans who are palliative and wish to be cared for at home.
This grant will fund a community organisation to project manage the delivery of the PMCP Project across the ACT. The successful organisation will be required to:
- design and deliver the PMCP project
- manage a grant process to engage participating pharmacies
- administer grant management and oversight
- establish and run a PMCP project community of practice (CoP)
- design an evaluation framework and collect and analyse data.
The successful community organisation will have experience working with primary health providers, managing communities of practice, and an understanding of the palliative approach.

This grant is for non-government organisations and/or service providers located in the ACT.
What you get
The total funding for this grant opportunity is up to $922,000.00 (GST exclusive, plus indexation), across a four-year funding period (2026-2030).
This funding is to be split between the costs associated with managing the delivery of the PMCP Project, and grant funding to engage seven community pharmacies (up to $20,000 (GST excl) per pharmacy, per year).

- have an account with an Australian financial institution, and be one of the following entity types:
- A company incorporated in the ACT under the Associations Incorporation Act 1991
- A company limited by guarantee and incorporated under the Corporations Act 2001 (Commonwealth)
- An incorporated trustee on behalf of a trust
- An incorporated association
- A partnership
- A joint (consortia) application with a lead organisation
- A registered charity or not-for-profit organisation
- Publicly funded research organisation
- are financially viable to deliver the project over the term of the grant.
- meet the criteria under ‘Eligible services’ in the Grant Requirements document.
- are compliant with ACT policy priorities, and where applicable Secure Local Jobs Code (SLJC) certification. Where SLJC certification is not applicable, it is highly recommended that you are SLJC certified or working towards certification.
You cannot apply if you are:
- a Commonwealth, state, territory or local government agency or body (including government business enterprises)
- an individual
- an unincorporated association
- an overseas resident or organisation.
What you can do
Your application must align with the grant funding priorities listed in the grant guidelines.
If successful, you will be required to lead all elements of project management to deliver the PMCP Project across four years. Specifically, you will be required to:
- design and develop the PMCP project
- recruit and onboard participating pharmacies
- establish and ongoing management of PMCP Community or Practice (CoP)
- manage and oversee the grant
- design evaluation framework and collection data to evaluate effectiveness of PMCP Project.

All applicants will be notified by email that their application has been received.
Assessing your application
All eligible applications will be assessed by the Grant Assessment Panel, managed by the Health and Community Services Directorate.
Once the Grant Assessment Panel has finalised their assessments, the Chair of the Panel will provide recommendations to the delegate for their approval.
If you are not successful you will be formally notified once a formal agreement with the successful applicant is in place. You can request a formal debrief, by emailing AEOLPolicy@act.gov.au within 5 working days of being notified. The business unit responsible for the grant will respond to your request via email as soon as practicable.
If you get the grant
The successful applicant must enter into an Agreement. This will be by a Letter of Offer - Deed of Grant, with the Health and Community Services Directorate.
The Letter of offer will include:
- The grant we are offering
- How to accept an offer
- Details on the funded grant amount
- The purpose or activity for which you have the grant
- The term of your funded activity for which you must expend the grant amount in full
- How to acquit your grant funds when you have completed the project.
