Summary

The ACT Dementia Health Experts Network, supported by the Health and Community Services Directorate, has developed an Evidence-based Framework for Dementia Care in the ACT. It is an initiative of the Health and Community Services Directorate as part of the ACT Government’s Better care of older Canberrans initiative.

Informed by extensive research, data analysis and a carer-led co-design process, it outlines four priority concepts to guide improvement in care and outcomes for people living with dementia and their carers. The concepts are:

  • Dementia Navigator to support care and system navigation, providing a single, trusted point of contact throughout the dementia journey.
  • Improved access to planned and emergency respite services to support carers and alleviate carer burden.
  • Dementia-specific community crisis support services to reduce avoidable emergency department presentations and hospitalisations.
  • Integrated dementia care hub to deliver multidisciplinary care, research and innovation, and provide support for complex cases.

The framework provides an opportunity for the ACT to become a leader in creating safe, inclusive, and supportive care environments. It will help guide the development of a connected health and aged care system that responds to the needs of people living with dementia and supports those who care for them.
