Healthcare facility incident notification
Healthcare facilities must report certain events to the ACT Chief Health Officer. This is a requirement under the Healthcare Facility Code of Practice 2021.
Notifiable incidents
You must tell the Chief Health Officer within 2 business days of:
- the transfer of a patient to another healthcare facility because of an injury or iatrogenic condition sustained during treatment
- any critical incident that might place systems or people at risk such as:
- flood
- fire
- building or structural collapse
- serious equipment failure or shortage
- sustained power or water failure
- the unexpected death of a patient
- any incident in the Australian sentinel events list.

How to notify
If you are a healthcare facility licence holder use the notifiable incident form (licensed) [DOC 141 KB].
If you are a public healthcare facility use the incident notification form (non-licensable) [DOC 144 KB].

How to report
You must prepare a report within 100 days to give further details of the incident.

You should also do a review after 6 months to see if recommendations:
- are in place
- are effective.
