Healthcare facilities must report certain events to the ACT Chief Health Officer. This is a requirement under the Healthcare Facility Code of Practice 2021.

Notifiable incidents

You must tell the Chief Health Officer within 2 business days of:

  • the transfer of a patient to another healthcare facility because of an injury or iatrogenic condition sustained during treatment
  • any critical incident that might place systems or people at risk such as:
    • flood
    • fire
    • building or structural collapse
    • serious equipment failure or shortage
    • sustained power or water failure
  • the unexpected death of a patient
  • any incident in the Australian sentinel events list.

How to notify

If you are a healthcare facility licence holder use the notifiable incident form (licensed) [DOC 141 KB].

If you are a public healthcare facility use the incident notification form (non-licensable) [DOC 144 KB].

You must prepare a report within 100 days to give further details of the incident.

You should also do a review after 6 months to see if recommendations:

  • are in place
  • are effective.
